HOW DO I....
OBTAIN A REPEAT PRESCRIPTION?

A repeat prescription may be issued to you if you require regular medication, although regular reviews are essential. Please allow at least 48 hours (excluding weekends and bank holidays) for the prescription to be prepared. It is the policy of this practice and NHSERY to issue repeat prescriptions for 28 days' supply at a time.

REQUESTS

  • Tick the box on the prescription slip and bring it or post it to your usual surgery. Please enclose an SAE if you wish the prescription to be posted.
  • Request your prescription on the internet either via the practice website or directly on https://access.e-mis.co.uk
  • Leave your request on the answerphone. If medication is to be collected from Snaith, telephone 0844 477 8699; from Rawcliffe, telephone 01405 839201 (after 11.00am if there is no query). Please state your computer number and what items you require. If you have a query about your medication or repeat prescription leave your name and contact number and a dispenser will phone you back.

COLLECTION

At Snaith repeat prescriptions are NOT dealt with during peak surgery times. Please collect, if possible, between 10.30am and 3.30pm. Rawcliffe patients can collect at any time whilst the surgery is open.

If you are a non-dispensing patient and would like to collect your medication from Snaith or Eggborough Pharmacies, the script will be collected by the pharmacies. Please apply at either pharmacy to join their scheme.

MEDICATION DELIVERY

The practice offers a drug delivery service to those dispensing patients who have difficulty collecting medication either through lack of transport or ill health. Please ask at the dispensary for further details.

DISPENSING

We are willing to dispense for all patients living more than 1.6km from a chemist but NHS rules prevent us from dispensing to any patient living nearer.

The dispensary is open at the same times the surgery is open. The surgery opening hours are clearly displayed on the glass door of the entrance. If medication is urgently required when the dispensary is shut, details of the nearest available pharmacy can be found by ringing NHS Direct on 0845 4647.

HOW DO I....
OBTAIN SICKNESS CERTIFICATES?

If you are absent from work due to illness for less than seven working days you need to obtain a form SC2 from your employer. If, after seven days, you are still unwell you will need to see a doctor who may then issue a medical certificate.

If your employer asks for a certificate for an illness lasting less than seven days, a private certificate can be issued but a fee is payable for this. Form SC1, required by the self-employed, can be obtained from the surgery.
